Here is the English translation of the French document "Demarches sociales". It is up to date as at end January 2013.

It is a comprehensive 96 page document which covers most aspects of health care. It has a very useful appendix with telephone numbers, websites etc. together with a glossary and a list of acronyms. This is the Table of Contents which will give you an idea of what is included:
