Here is the link to the page on the INCa website which sets out the details about the reimbursement of travel costs when undergoing treatment for ALD (illnesses of long duration). In a nutshell, as long as your doctor prescribes a particular type of transport, this should be reimbursed by your Caisse d'assurance maladie. It is the doctor who decides that you need, for example, a taxi rather than the train. Your doctor should sign a doucment for you effectively authorising the type of transport used. In the case of long journeys (over 150 kms each way), this authorisatiion should be sought in advance to avoid future problems. Journeys by boat or plane also need to be authorised in advance.
Note however, if you CHOOSE to be treated outside your department then you must also get authorisation in advance to be sure that the travel costs willl be reimbursed.

And this is a link about travelling abroad for treatment. Again, authorisation before the trip is necessary.
